Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff.

APPOINTMENTS

Appointments

8am – 6pm.  This surgery is run on an appointment system. If you need to see a doctor and you do not have an appointment we will see you as soon as possible and emergencies will be attended to promptly.  If you require a longer consultation please notify the receptionist when making your appointment.  If for any reason you cannot keep your appointment you must call and let our receptionist know, as other patients waiting for an appointment can be notified and booked into the time. Saturdays after 1pm no appointment needed, and Sundays and Public Holidays no appointment needed. Pap smears need a separate appointment and Receptionist should be informed when the appointment is made.

Please inform the receptionist of your preferred GP at the time of making your appointment.  You will be informed if your preferred GP is unavailable and given the option to nominate another GP for your consultation.

Home Visits

In circumstances where a patient is unable to attend the surgery, please contact us for Heaths Rd on 9748 4188 or Sayers Rd on 97485088 to arrange a home visit.

BILLING

BULK BILLING: Services at this clinic are primarily bulk billed however a fee may be charged at the time of consultation for the following items

FEE SCHEDULE (Fees other than bulk billing):

Medical Forms

• Pre Employment Fitness Certificate
• Centrelink Forms
• Drivers Licence Renewal
• Insurance Forms

(the amount charged will depend upon the amount of information required and time taken to complete the report at the doctor’s discretion)

Please note Medical Forms will not be completed at the time of consultation without prior approval from the Doctor.

Pathology Tests
Thin Prep Pathology tests will incur a seperate fee. You will be notified of the charge at the time of consultation.

Workcover Consultations
All workcover consultations must be paid in Cash/Eftpos/Credit Card by the patient(unless a valid claim number is given at the time of the consultation.)

Westgate Medical Centre provides a locum service for after hours emergencies.
Please call 132660 if your emergency is outside of clinic hours.
